Flying Tiger Flight 66 Accident in Kuala Lumpur, Malaysia on 18 February 1989, approximately two months after Fred Smith announced to the world Federal Express was buying Tigers. This CVR recording is chilling, including the crew's last words followed by the sound of the explosion. This video is an excellent training resource, but I cannot help but wonder what role fatigue played in the accident.
